Incapacity-for-work assessment – a step-by-step explanation

If you claim an Anw survivor benefit and state that you are at least 45% incapacitated for work , the Employee Insurance Agency (UWV) will assess whether that is in fact the case.

This page gives a step-by-step explanation of what this assessment entails.

The 4 steps of the claims procedure

  1. Step 1. We will telephone you

We will telephone you to explain what you can expect. For example, the UWV often needs additional information from you to conduct its assessment, even if you are already receiving a benefit from the UWV.

  2. Step 2. We will send you a form

We will send you a form. If you need any help filling in the form, please get in touch with us. We will be happy to help you.

  3. Step 3. You must send the completed form to the UWV

You should send the form to the UWV within 10 days. This will enable the UWV to complete its assessment within 6 weeks. If you send the form later, it may take the UWV longer than 6 weeks to complete its assessment. If the UWV has any questions while carrying out its assessment, they will contact you.

  4. Step 4. We will send you our decision

The UWV will let us know when they have completed their assessment. We will then send you our decision, which will state whether or not you are entitled to an Anw survivor benefit.
