Applying for an assessment of your Wlz insurance position
If you want to know whether you are insured under the Wlz scheme, for example because you have received a letter from the CAK or because your health insurer needs an official decision from the SVB, you can apply to us for an assessment of your Wlz insurance position.
If one of the situations described below applies to you, you do not need to ask us to assess your Wlz insurance position.
If you are doing an internship outside the Netherlands, or if you are doing voluntary work, working as an au pair or are doing PhD research and you have not received a letter from the CAK, you do not need to request an assessment of your Wlz insurance position. Simply contact your health insurer.
Can you apply for a Wlz assessment for someone else?
You can only apply for a Wlz assessment for yourself or for your children under the age of 18.
Your partner or any children over the age of 18 will need to apply for a Wlz assessment themselves.

If you wish to apply for a Wlz assessment for a child under 18, you will have to send an application form by post.
Children over the age of 18 must apply for a Wlz assessment themselves.
